Walt Disney Guest Assistant Card Question?

Im going to Disney and asking for a guest assistance card. I have Epilepsy and will take a paper from my doctor(It says DX: Epilepsy, my name, and the doctors signature) if I just go and say "I need a guest assistance pass, I have epilepsy(and show them my papers), which GAP will they give me?
